To submit an Individual Rights request, please complete the information requested below.

The personal data required by this form is necessary to enable the Everest companies to process your Individual Rights Request. The information you supply will only be used for the purposes of identifying the personal data you are requesting and for responding to your request, in accordance with applicable privacy laws, including but not limited to the California Consumer Privacy Act of 2018, as amended by the California Privacy Rights Act of 2020 ("CCPA"). The information provided may be shared for processing and preparing a response and it will be stored securely until the relevant procedures are completed.

We will need to verify the identity of the consumer in order to complete this request. Based on the consumer’s relationship to Everest, fill out the pertinent fields below which will facilitate our search for the consumer information and help expedite this request.

Declaration

I confirm that the information provided on this form is correct and that I am the individual whose name appears on this request and am entitled to make this request.

What happens next?

If your request is valid, we will acknowledge your request in writing and provide you with a reference number relating to your Individual Rights Request then start processing your request.

If your request is valid but we are unable to verify your identity, we will advise you of this and request additional information.

We will make every effort to process your Individual Rights request as quickly as possible within 30 calendar days once we receive sufficient information to do so.

However, the Everest Companies may require additional time if you do not provide sufficient information to verify your identity, sufficient information to locate your data, or for other reasons permitted by applicable law.

We will inform you of the reasons for the delay, to the extent applicable.

If you have sent us an invalid request, we will return your request along with any enclosures and advise you why your request has been rejected.

Note that all information disclosed pursuant to a Individual Rights Request is a file copy; originals are not provided.
